  • Upon booking a medical appointment, please be sure to fill out and submit the insurance information.  This ensures that we can process paperwork and bill your insurance directly instead of sending the bill to you.
  • Cosmetic consultation appointment fees are not covered by insurance.  However, all treatment fees include a free initial consultation & post treatment followup.
  • All cosmetic treatments are given by fully qualified physician or physician assistant with formal medical training and licensing.

After booking an appointment, you will receive a reminder 48 hours prior your appointment.

If you cannot make your appointment, we ask that you call or email us to cancel or reschedule at least 24 hours in advance.  

No-shows or last minute cancellations for medical appointments will be assessed a $75 fee.  Cosmetic Consultation and SculpSure Consultation fees are not refundable for no-shows or cancellations less than 24 hours of appointment time.


Arrival
Please arrive at least 10 minutes prior to your appointment time to checkin and relax for your appointment. 

New patients are required to complete a medical intake form. 

For cosmetic treatments, consent forms must be completed even if you are a regular patient. The medical intake form and consent forms contain questions relating to your current medical care and previous medical history. In the case of cosmetic treatments, you are required to complete all questions to allow the clinician to assess your suitability for treatment. All information provided on the consultation and consent forms will be discussed between you and your clinician. This confidential discussion will enable both to ensure that your goals are understood and achievable. During your consultation we will also thoroughly explain the treatment, ensure you understand the advantages and if there are risks associated with the treatment and assess your suitability. If you are unsuitable we will explain the reason why and what your options are and offer an alternative where possible. Lumos Dermatology® complies with HIPAA and all information given will be held in the strictest of confidence and under no circumstances whatsoever, be passed onto any third party.

Late Arrivals
 
Our appointment system is designed to allocate the correct time for your treatment. Unfortunately, a late arrival will result in a reduced appointment time or you may be required to reschedule your appointment. If you find yourself running late, please call us, 646-820-1716.
 
Cancellations

Lumos Dermatology® accepts cash, Visa, MasterCard, American Express and Discover.  Checks are not accepted.
 
Treatment Suitability

Lumos Dermatology® will always assess whether or not a treatment is suitable for you during your consultation. If you are unsuitable for treatment, we will inform you as to why and discuss your options and offer an alternative where possible.

Lumos Dermatology® cannot guarantee that you will achieve the same results as the before and after photographs on any promotional material including but not limited to the website, treatment brochure or leaflets.

Limited Liability
No patient will receive any form of treatment unti Lumos Dermatology® has been provided with all medical details of the patient which shall be provided in writing and form part of the medical intake form and treatment consent forms which will be provided prior to your consultation or treatment. It is the patient’s sole responsibility to ensure that they have provided Lumos Dermatology® with all medical details in writing prior to each treatment and such details shall form part of these terms and conditions.

The client hereby undertakes to comply with all instructions and/or recommendations given to them by Lumos Dermatology® regarding the aftercare of any treated area and Lumos Dermatology® accepts no liability of any kind incurred as a result of the patient’s failure to follow such instructions and/or recommendations.

The terms and conditions along with the medical intake and consent forms represent the full agreement between Lumos Dermatology® and each patient.
